Below are some forms to download

1.Request For Baptism Forms. Please phone the Parish Office to book a baptism and to check available dates. Baptisms currently take place in the Parish Pastoral Centre (Our Church is currently closed for essential repairs) every Sunday of the month at 12.30 PM. until the we have completed the large number of baptisms awaiting from 2020-2021.   See Guidelines for Baptism

To request a Baptism Certificate one must first complete a Baptism Cert Request Form (in order to be compliant with the Data Protection Regulations). Form below

2.Statement of Freedom to Marry Form  statement-wedding

This form is one of the documents required when preparing Marriage Papers. It may be signed by a relative and must be witnessed by the relative’s local priest. Other documents required are a newly issued baptism cert, confirmation cert, a pre-marriage course cert (issued after completion of a pre-marriage course) and finally a pre-nuptial enquiry form, which takes about 30 minutes to do and so it is necessary to make an appointment to see the priest or deacon in the parish that you currently reside. 3. Funeral Readings and Prayers

6. Setting up a Standing order for Collections

This can be done online through the Donate page or by downloading the Standing Order Form and returning to the parish
